2026 Insurance Booking Notice

New Patient

​

Insurance information must be provided upon booking confirmation or earlier. If not submitted at the time of booking, you have 3–5 days to provide the required insurance details for verification.

​

If insurance information is not received within this timeframe, the insurance appointment will be automatically canceled to allow the time slot to be available to other patient. You will have the option to rebook under out-of-pocket services.

​

Bodyworks Massage operate on a trust-based booking system and do not require a credit card on file. Patient, are expected to book honestly and provide accurate contact and insurance information.

​

Missed appointments or repeated non-compliance may result in future bookings being out-of-pocket only.

​

Bodyworks Massage's policies exist to protect the quality of care, time, and availability for those who truly need it.

Insurance & Booking Policy 

​

Insurance appointments require benefit verification prior to service.
A valid insurance card must be provided. Appointments booked under insurance without verification may be canceled.

​

Returning patients: If you have new or updated insurance, please provide your current insurance information prior to your appointment to avoid billing issues.

​

Please note: At the beginning of the year, most insurance plans reset deductibles. If your deductible has not been met, you are responsible for the full out-of-pocket cost at the time of service. Insurance coverage does not guarantee payment.

Missed appointments or late cancellations may result in a no-show fee billed directly to the patient. Repeated no-shows may require future bookings to be out-of-pocket only.
